Janet Lawless and Mahdi Ghararizadeh Mahani and 6Tigers Academy agree to provide Martial Arts and/or Self Defence
and/or Fitness instruction (the Program) to the student.
In consideration of this agreement and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I agree to release and indemnify Janet Lawless and Mahdi Ghararizadeh Mahani and 6Tigers Academy (which term includes its instructors, assistants, successors and assigns) and those persons participating in the activity with me/my child from any and all claims, actions, causes of actions, suits, covenants and demands whatsoever arising from any injury or harm to myself/my child caused while I/my child is attending the Program both on and off the premises and any loss or damage to personal effects that should occur during the Program.
I understand that there are risks and dangers inherent in participating and/or receiving instruction in martial arts, fitness and methods of self-defence. I understand and agree that this Release and Indemnity will be binding on me, my spouse, my heirs, my personal representatives, my assignees, my children and any guardian for my children.
I acknowledge that I have read this Release and Indemnity and that I understand the words and language in it. I have been given sufficient time to consider my actions and have sought and received independent legal advice as I deemed appropriate with respect to this matter and the terms of this document. I acknowledge that I sign this release and indemnity voluntarily. I have been advised of the potential dangers incidental to participating and/or receiving instruction in the Activity.
I, on behalf of myself and/or my minor child, agree to allow 6Tigers Academy Incorporated, its directors, agents, officers, principals, employees, contractors and volunteers, to use the picture(s), film and / or likeness of me and/or my child for advertising purposes, without any compensation to me or my child. In the event I choose not to allow the use of the same for said purpose, I agree that I must inform 6Tigers Academy Incorporated of this in writing.
This Release and Indemnity shall be deemed to have been made in and shall be construed in accordance with the laws of the Province of Ontario.
